Planning a ministry event? Fill out the various forms below based on your event needs. Don't forget to add to the Google calendar.

HEADS UP FORM
This is meant to alert all ministry areas about your event to prevent overlap with scheduling. Even if you don't have all the details of your event figured out, please fill out the form with what you do know. :)

PROJECT REQUEST FORM
This is for all things design, print, web, and promotions related to your event. The Publications team needs this 6 weeks before you want to start promoting so most likely 3 months before your event. :) Even if you don't have all the details, send in a PRF so it's on their radar.

CHILDCARE FORM
Don't forget about the kiddies! The childcare form is for any needs related to caring for littles (Birth-5th grade). This is needed 1 month before your event.

WORSHIP REQUEST FORM
Testing...1, 2, 3...testing. If you want any worship-team led music at your event, let the worship team know 3 months in advance.

FACILITIES
Our facilities team is amazing! To help them help you, put your facility needs (tables, chairs, water, etc) in the calendar. Also, email the facilities team with details of your event at [email protected].

SECURITY
One security officer is required for every 100 people on our campus with a maximum of three officers. If you are offering childcare, one officer will be dedicated to that area. Email Sean Cobb at [email protected] with the date/time of event, expected head count, and childcare status. Sean will coordinate with our security team and respond with final approval of your request.

TECH REQUEST
Lights, camera, slides ... do you need one of these at your event? Our super talented tech team can help you plan a stellar event. Whether you have a trained volunteer to run lights, slides, or camera or need them to hire someone, you can get that setup on this form.

BACKGROUND CONSENTS
All volunteers (18 and up) need to fill out a consent for background screening. Just turn in the forms to Sagan Ezell, our database manager, and don't forget to add your ministry name at the bottom of the form.

NEW HIRE DOCS
Whether you are adding a part-time or full-time person to your team, this has all the docs you'll need as well as a handy checklist for what to do. Don't forget to email a PDF of the completed & signed docs to [email protected].

SECURITY INCIDENTS
Complete this form any time an injury happens on our property whether it be a team member, church member, etc. This can also be used for non-injury related incidents (ex: trespassing, vandalism to church property, theft, etc.). Hard copies will be kept in the Guest Center. Completed forms can be scanned and sent to [email protected].
